If David's parents were doctors why do you think they went to the Smoke?
I think David's parents ran away to find the smoke but ended up liking it and stayed. Because, they were doctors I suspect they helped to get the smoke going a little better. I think this because of sentences in the chapter like "My parents were runaways not me."

I think they actually started it because they didn't want David to be a city-dweller. When David told Tally that he didn't usually tell people the secret of his parents, I think he left something out: his parents' reason for leaving. :)
